Sports Physicals

 

Student athletes are required to complete an annual sports physical examination performed by a medical doctor (MD or DO), nurse practitioner (NP) or physician’s assistant (PA) prior to participating in competitive sports during their middle and high school years. Students are to turn in their sports physical along with their Athletic Health Clearance Packet to their Athletic Director. Roseland Charter advises students to complete their annual physical as soon as possible after June 1st.

 

Please follow the link below to view additional resources where your student may complete their physical examination prior to the start of their school’s sports programs. Please contact the organizations listed on the document for appointment availability and additional information regarding medical services (i.e. sports physicals) and billing.
