Enrollment Information

If you are interested in enrolling your child at one of our schools, please fill out the enrollment form for the school you are requesting, bring all requested items and return your enrollment packet to the school office of choice.  You may stop by one of our schools for a copy of the enrollment packet available at the school office or download it from the grade level section below.You will need to have the following items to enroll your child at one of our schools.

1. A completed Enrollment Form
2. Proof of residency (current-within the last 30 days, your address on a rental agreement, purchase agreement, utility bill)
3. Proof of student's age (birth certificate, baptismal certificate, or other accepted form of proof of age)
4. Proof of immunization records (DTP 5, Polio 4, Hepatitis B, MMR 2, Varicella 1)
5. Inter-district form (Only if out-of-district)
6. Special Education Services Information (Copy of IEP, if applicable)
7. Physical Exam (Recommended) - Appointment card will be accepted
8. Dental Exam (Recommended) - Appointment card will be accepted
